How to Apply
The Case Manager will assist clients experiencing homelessness or at risk of homelessness in applying for Social Security disability benefits. The Outreach Specialist is responsible for engaging with individuals experiencing homelessness to build trust and connect them essential services. This includes conducting street-based outreach, educating clients on available resources, and facilitating connections to housing and mental services.
